Prescription Medicines Information

All prescription medicines require a signed prescription from your veterinarian that must be posted or faxed to us before the order will be dispatched.

Follow these steps once you have placed your order:

1. Download and print the form using the link above.
2. Have the form filled by your vet and enter your Vetmedsplus order number in the top right hand corner.
3. Post the form to the address below or fax it to 01491 577 844.

Please note that failure to indicate the Vetmedsplus order number on the form will result in a potential delay of delivery.
